Rates & Insurance

Cognitive rehabilitation, neuropsychological testing and psychological evaluations are usually eligible for reimbursement from most health insurance companies.

Rates
 

 

​I tailor my approach to each individual client. Cognitive rehabilitation typically lasts 6 to 8 weeks, with a 55-minute weekly appointment. Neuropsychological testing and psychological evaluations usually require between 6 and 14 hours to complete.

My rates are:

  • Cognitive rehabilitation: $210 an hour

  • Testing and evaluations: $250 an hour

 

Please provide full payment or co-payment at the time of service. I accept checks, cash, and credit cards. The fee for missed appointments or late cancellations is 50% of the hourly rate. Insurance will not reimburse this charge.

Insurance
 

 

I am an in-network provider with Legacy+, PacificSource and First Choice Health. If you are a member, I will collect a co-payment at the time of service and bill your insurer for the balance.

 

If you have another health insurance, I will collect full payment at the time of service. If your carrier offers out-of-network reimbursement, I will provide you with a detailed invoice, also known as a superbill, so you can submit a claim directly to your carrier.

Most health insurance providers offering out-of-network benefits will reimburse between 30% and 70% of the total service costs. Please contact your insurer directly for more details of coverage. Your insurer may ask for a list of CPT codes. Please reach out to me if you would like that list.

Self-Pay & Good Faith Estimate
 

Though cognitive rehabilitation, neuropsychological testing and most psychological evaluations may be eligible for reimbursement from your health insurance company, clients may also choose to self-pay. If you are uninsured or plan to self-pay (without submitting a claim to your health insurance), you have a right to a Good Faith Estimate.

The Good Faith Estimate provision of the No Surprises Act grants clients the right to predictable healthcare charges. I tailor neuropsychological testing and psychological evaluations to each client and can provide an estimated range for charges once I complete your intake.
